For many families, turning 21 doesn’t mean care needs disappear, it means they evolve. Our transitional care program provides continued medical and developmental support for individuals who have aged out of pediatric services but still require skilled nursing or therapeutic care.

Designed as a smooth transition from our pediatric programs, our transitional care offers a familiar, structured environment for individuals who have previously attended Caring Corner or another Pediatric Day Health Center (PDHC). Participants receive ongoing support from our nurses, caregivers, and therapists in a setting that prioritizes dignity, safety, and personal growth.
